Sports Physicals

All students, grades 7-12, who participate in school sponsored sports must have a sports physical for the 2018-19 school year.   Students in grades 5 & 6 may have a physical, but is NOT required.  Physicals MUST be completed and turned in to the school or coach before the first practice of the season; football practice starts August 6th and volleyball starts August 13th.  Student athletes MUST have 9 practices completed prior to their participation in a contest, students CANNOT practice without a physical and practices do not count until the physical is turned in. 

Below is a list of local providers, however you are free to go to any provider you choose.  The NDHSAA Preparticipation Physical Evaluation Form MUST be used and returned to the school or coach upon completion.

McKenzie Healthcare Systems - times and dates are set as designated per the diagram.  If you cannot make the dates designated by McKenzie County Health Care, physicals will be done at Urgent Care located in Emergency Department.

When you go to your appointment, be prepared by having a copy of the physical form with you.  The link provided below and will be available at the school office.
